We are searching for a nationally certified Nurse Practitioner/Physician Assistants to join our team. In this field-based role, you would provide a holistic approach to caring for our aging residents in long-term care facilities. Key responsibilities of the Nurse Practitioner/Physician Assistants include:

Providing direct care to aging residents in long-term care facilities

Collaborating with primary care physicians, facility staff and patients’ families

Participating in care conferences with other providers and patient family members

Working with the facilities utilization review process

Identifying patients’ behavioral health issues

Educating nursing home staff, physicians and administration

Coordination of care for patients

Requirements:

Nationally certified Nurse Practitioner or Physician Assistant status

Certification in Adult, Family or Gerontology

Licensure by the State of MA or ability to obtain licensure.

Proficiency in Computer Skills

In addition, assets that would be beneficial include:

Clinical experience

Long-term care experience

Knowledge of mental health issues

A background in managed care

Familiarity with Medicare and ICD-9 coding
